Departing Traveler Information

    1. All travelers ages 2 and above must complete page 2 of the CDC Combined Passenger Disclosure and Attestation and a Traveler Health Declaration.
    2. Fully vaccinated travelers must provide appropriate proof of vaccination and provide proof of a negative COVID-19 test result, taken no earlier than 1 day prior to departure.
    3. 3. Unvaccinated (or not fully vaccinated) travelers must provide proof of a negative COVID-19 test result, taken no more than 1 day prior to departure. On USAG Humphreys, the tests can be administered at the COVID Reception Center (Bldg. 118), DSN: 737-1230, from 0800-2300.
    4. Recently recovered from COVID-19 travelers must provide proof a letter from a licensed provider stating that traveler is cleared to travel.

Any non-citizens, non-immigrants travelling to the U.S. (CDC Order dated Oct. 25, 2021)

    1. Traveler must complete pages 2, 3, & 4 of the CDC Combined Passenger Disclosure and Attestation and a Traveler Health Declaration.
    2. Must be fully vaccinated or exempted through one of the several methods, including, but not limited to the following: 1) Children under 18 years of age (must agree to be tested 3-5 days after arriving in the U.S. and self-quarantine for the first 7 days upon arrival). 2) Non-citizen members of the U.S. Armed Forces or spouse or child of a member of the U.S. Armed Forces.
    3. Additional exemptions are detailed in the CDC Order dated Oct. 25, 2021.

Guidance Remaining in Effect

    1. No testing requirements for children under 2 years of age.
    2. Testing can be waived for redeploying military members by the first O-7 or civilian equivalent in the chain of supervision. This exemption must already have been provided to USTRANSCOM at the time of check in via email (note: the document must also be readily provided when requested by a passenger service agent).
    3. Space-A travel is still largely restricted, however certain categories remain eligible (See Space-Travel Limitations Due to COVID-19). In addition to the eligible categories, exceptions to policy may be granted in writing for those with compelling cases where travel is (a) necessary for humanitarian purposes or (b) warranted due to extreme hardship. Those travelers with exception to policy documents must readily present their document to the passenger service agent for travel aboard military airlift.

For Patriot Express Travel, individuals unwilling to complete all required forms and/or provide all required documentation will be denied travel.
