Command Sponsorship

Bring your family to Korea! The Republic of Korea is a great place to serve and grow personally and professionally, for both Soldiers and their Family members.

Being approved for Command Sponsorship means that your family members are able to accompany the Soldier Outside the Continental U.S. (OCONUS) for the length of his or her tour. The Soldier must apply for Command Sponsorship. The process includes an EFMP screening of each family member and approval from your Soldier’s gaining command. This process can be initiated 180 days prior to Soldier’s PCS Report date to allow for Command Sponsorship Orders. While Command Sponsorship of dependents is not guaranteed, all Soldiers should consider applying.

While each losing installation may be slightly different, after receiving a Request for Orders (RFO) or assignment orders, the Soldier should immediately request Command Sponsorship through the Military Personnel Division (MPD) Personnel Processing Branch.

The following forms and requirements must be submitted through the Soldiers servicing MPD no more than 180 days from or 30 days to the Soldiers report date:

  • Out of Country Forms (currently not in Korea)
    • DA Form 4787 Reassignment: Provided to you by your reassignments work center
    • DA Form 5888 Family Member Deployment Screening Sheet: Provided to you by your reassignments work center.
    • DD 2792 Exceptional Family Medical Summary: Provided by screening agency if required.
    • DD 2792-1 Special Education Early Intervention Summary: Provided by screening agency if required.
    • Sex Offender Declaration Memorandum: Example
    • NOTE: 2792 and/or 2792-1 with IEP are only required if DA Form 5888 is annotated “Consideration Warranted.""

    Requests being submitted when the Soldier is on orders to Korea must be submitted by the Soldiers losing installing MPD <180 days but >30 days to the Soldiers report date to usarmy.humphreys.8-army.list.csp-korea@mail.mil

  • In Country Forms (currently in Korea)
    • DA Form 4187: Example
    • Original PCS orders bring Soldier to Korea and Pin-point Orders
    • DA Form 5888 Family Member Deployment Screening Sheet: Initiated by the Soldier and authenticated by the unit S1. Get Form
    • DD 2792 Exceptional Family Medical Summary: Provided by screening agency if required.
    • DD 2792-1 Special Education Early Intervention Summary w/IEP: Provided by screening agency if required.
    • Current Leave and Earning Statement (LES)
    • Travel Voucher Advice of Payment (AOP) from Soldier PCS to Korea. If a Soldier has been in Korea more than six months he or she must submit a travel voucher summary from finance. DD Form 1351-1 will not be accepted.
    • DD 1172 Application for DOD CAC Card DEERS enrollment from the DEERS office for Soldiers with dependents
    • NOTE: 2792 and/or 2792-1 with IEP are only required if DA Form 5888 is annotated “Consideration Warranted".

  • Track your Command Sponsorship request
    • Already submitted a Command Sponsorship Packet and want to check the status?
    • After receiving your CSP tracking number from your local MPD, go to: www.us.army.mil/suite/page/671823
    • After logging in through AKO, use the number provided by MPD to track the status of your request.

How long does the application take?

The entire application process can take up to 30 days from completing the required forms and family EFMP screenings (and enrollments if needed) to the review of your completed application.
